Cold starting question

When I start my '02 C2 Cab when it is dead cold, it will idle very rough and at about 700 rpm. If you feather the throttle a little and get it to rev past the "loaded up" feel then it runs fine and idles like normal. It doesn't do this all of the time, but it does it enought to be bothersome. Once it is started and it gets past this point it won't do it again until it is stone cold. This just lasts until you rev past the loaded up feel.

Any thoughts on where to look? It almost acts like it the plugs are fouled and then they clean up. The car has 35,000 miles.

I would start with a intensive throttle body cleaning and await the outcome, before i take further steps.

I tried it again this morning and it was like before, but I immediately shut it of and re-started it and it runs perfect.
